How do we collect information from you?
We obtain information about you when you use our website, for example, when you contact us about our services and when you register as a patient or client at our Neuroactive Fitness Clinic.
Neuroactive Fitness collects personal information about you whenever you use our services. Some of this information you give us directly when, for example, you visit one of our clinics. Other information is collected indirectly, for example when we receive your details from a third-party referral company.
What type of information is collected from you?
When browsing our website, the personal information we collect includes your IP address and information regarding what pages are accessed and when. If you attend our clinic, your name, address, email, date of birth and medical history will be recorded and held for 6 years.
Additionally, when assessing and working with you, particularly when delivering exercise and personal training services, we will with your verbal permission, capture video and photographic data which will be treated with equal respect and held in our clinic systems to allow us to monitor your progress and enhance the level of service we provide. We typically use this data to analyse your movement, gait and to identify and monitor neurological symptoms which can often be more effectively recorded and monitored during physical activity and exercise.
If you make a payment online your card information is not held by us, it is collected by our third-party payment processors, who specialise in the secure online capture and processing of credit/debit card transactions.
How is your information used?
● To provide and fulfil Services between yourself and Neuroactive Fitness and that of your insurance provider
● To refer you onto a Consultant or GP, where you have given your consent
● To notify you about changes to our Services
● To provide customer support, e.g. appointment reminders
● To gather analysis or valuable information so that we can improve our Service
● To monitor the usage of our Services
● To detect, prevent and address technical issues
You will need to give us permission to email you with news, special offers and general information about other services we offer that are like those that you have already purchased or enquired about. We will obtain your express permission to use any information for future marketing. All marketing emails from Neuroactive Fitness will allow you to opt out of further mailings. If you have opted-in to marketing emails then you will continue to receive these, until any such time you wish to unsubscribe.
How long do we store your data?
Who has access to your information?
We share your data as needed amongst authorised employees, agents and contractors providing services to you at the Neuroactive Fitness Clinic to support your treatment, rehabilitation, fitness programmes and to improve the overall service we provide. We do not pass your information onto any other party without your explicit consent. If you are seeking treatment or exercise and your sessions are funded by a third party, we are required to pass information regarding your condition and treatment to them as a requirement for funding. All reports will be explained prior to being sent.

Security of Data
The security of your data is important to us: your personally identifiable information is kept secure. Only authorised employees, agents and contractors (who have agreed to keep information secure and confidential) have access to this information.
We store personal information in a specialist clinic records management system and occasionally hard copy files that are kept in a secure location. This includes any video footage and photographs you allow us to take during your exercise or personal training assessments and sessions.
Personal information that we store or transmit is protected by security and access controls, including username and password authentication and data encryption where appropriate.
